Transfer To Gujarat

A person who is a Registered Pharmacist in other State Pharmacy Council and is now residing in Gujarat State and is intending to carry on business or profession of pharmacy in Gujarat, can apply for transfer of his registration to Gujarat state by submitting an online application.

The applicant should create an account in our website and fill the details and upload the required documents in "Transfer of registration" application on the dashboard.

The uploaded documents will be reviewed by council and the applicant can procced for booking an appoinment if the documents are found ok.

The applicant can pay the application fees online after original document verification on the day of appoinment.

The transfer of registration shall be subject to receiving "No Objection Certificate" from the council where he/she was previously registered.

List of Documents to be uploaded

1. If the name of the applicant differs than that in the acedemic record, you should upload - (a) Marriage certificate (For change in name of female applicants after marriage) ) or (b) A copy of gazette notification (for name changed in all other cases)

2. Proof of Birth Date & Birth Place (School / College leaving certificate / S.S.C. Board certificate or Birth Certificate from competent authority)

3. Proof of residence in the Gujarat
The applicant must upload a proof of residence in Gujarat state to get his/her registration transfered in Gujarat state pharmacy council
Valid list of documents as address proof

Electricity bill / telephone bill / water bill / gas bill in the name of candidate or parents / grand parents of the candidate
Voter Id
Aadhar Card
Rent aggriement
L.I.C. policy of the candidate
Election card of the candidate
Bank passbook
Tax Bill from the relevant authority of panchayat or Nagarpalika or Municipal corporation
Passport of the candidate

7. College bonafide / Course Completion certificate / college leaving certificate clearly showing period of study of the pharmacy course.

8. Degree/Diploma in Pharmacy or Pharm D Mark sheets of all semesters /years

9. Degree/Diploma in Pharmacy Certificate obtained from relevant University/Board of examination OR Provisional Certificate of the University/Board (if degree certificate is not received yet)

10. For Diploma in pharmacy : Practical Training Completion Certificate duly completed Section I-V with seal and signature of the principal of the concerned institute in Section-V of the Practical Training contract form
For B.pharm: 1 month industrial training post completion of B.pharm course
For Pharm D : Certificate of hospital training for period of 1 year.

11. Proof of Employment/Business of the candidate ((a) Employer's Certificate mentioning period of service, designation, head quarters, Salary etc. from relevant Firm/Company/Institution mentioning Drugs Licence Nos, if any ; or (b) In case of self employed persons, A self declaration on the letter pad of the relevant Firm/Company etc.
